When there are existing Actions for the Asset that the Defect is for, use the Defect Form, Relate Action Task to relate any number of those Actions to it:

•      If you know the exact Reference ID of the Action to be Related, enter it then click Find, or…

•      If you know when the Action was created, enter the From Date and To Date then click Find, to produce a list of Actions, or…

•      If you have a View of Actions set up, select it from the Available Filters picklist, click Find then, from the resulting list of Actions, select those that you want to relate the Defect to

•      Click OK

•      Alternatively, you can relate an Action to a Defect from the Action Form, Relate Defect Task, which lists uncompleted Defects on the Asset and its descendants, for you to select from

Note: from the Action Form, a Defect can be unrelated by selecting it in the Related Items tab then clicking the Unrelate Task.
